1. Login to the MyMoffitt Patient Portal - https://my.moffitt.org.
2. Click on the Info button on the lower right hand corner of the page.

3. Click the drop down arrow next to ©2018 Moffitt Cancer Center.



4. Click the Privacy Policy link or the Terms & Conditions link to view the respective documents.

Get Support ID

1. Login to the MyMoffitt Patient Portal - https://my.moffitt.org.
2. Click on the Info button on the lower right hand corner of the page.

3. Click the drop down arrow next to Get Support ID.



4. The Support Id is used to help our vendor trouble shoot an issue, if you receive an error message on a page.  The Info button should be clicked on the page where the error message was received; and then click on the Get Support Id drop down arrow to get the support id for the page that has the error.
