Appointments

View Upcoming Appointments and Instructions

  1. Login to the MyMoffitt Patient Portal - https://my.moffitt.org.
  2. Locate the Appointments drop down list on the left side of the page; and select View Upcoming Appointments from the list.

  3. Click the View Instructions link to see your instructions for your upcoming appointment.

  4. You may print your appointments by clicking on the Print button.

Request Change to Appointment

  1. Login to the MyMoffitt Patient Portal - https://my.moffitt.org.
  2. Locate the appointment you need to change and click on the Options drop down arrow and select Reschedule appointment.

  3. Complete the form and click the Send request button at the bottom of the page.

  1. Make scheduling changes at least 48 hours before your clinic visit.
  2. Clinic staff will contact you to reschedule your appointment.

Request Appointment Cancellation

  1. Login to the MyMoffitt Patient Portal - https://my.moffitt.org.
  2. Locate the appointment you need to change and click on the Options drop down arrow and select Cancel appointment.

  3. Complete the form and click the Send request button at the bottom of the page.

Request New Appointment

  1. Login to the MyMoffitt Patient Portal - https://my.moffitt.org.
  2. Click the Request Appointment link under the Appointments drop down menu.
